Telehealth Instructions

Is your upcoming appointment via telehealth, phone or video call? You will receive a pre-registration form via email one day prior to your appointment.

Please complete it in advance to expedite your telehealth appointment. On the day of your telehealth appointment, you will receive a call from a triage nurse who will inform you about scheduling updates or if your appointment is via phone or video. Telehealth cannot be done via landlines. At the time of your appointment, be sure you are ready and available to receive your call or video call.
